Sharing Your Needs

Welcome!

Do you have a condition such as dyslexia, ADHD, autism, hearing loss, a visual impairment, a long-term medical or mental health condition?

Does it have a substantial impact on your ability to carry out normal day-to-day activities 

If the answer to these questions is yes, we encourage you to tell us so that we can support you during your academic studies. 

 

Types of Support Available

Our advisors will meet with you to discuss your needs and arrange your support which can include:
  • University adjustments for learning and exams
  • Work placement adjustments, if applicable
  • Funded study skills support 

How Do I Share My Needs?

To start this process, you will need to share your personal needs to the University via the secure online ‘Disclosure of a condition’ form, which can be accessed via your ‘My application’ portal.

Any information you share with the team will be treated as confidential. You are welcome to phone or email the Disability and Dyslexia Service directly on 01243 812076 or disabilitydyslexia@chi.ac.uk to speak to us about your support.

You will need to provide suitable evidence of your need, such as a recent Educational Psychology Report or medical evidence from a doctor or another specialist, as appropriate, to support your request. 

If you have not already told us about your condition please complete the secure online ‘Disclosure of a condition’ form, which can be accessed via your ‘My application’ portal.

You can use this form to give us some idea about the sort of support and adjustment you might need at University. Completing this does not guarantee the support, but it can help letting us know what you think you might need so we can discuss this further.

You need to provide evidence of your disability or medical condition if you have not already done so. The form allows the upload of evidence in suitable format such as document, pdf or picture format (jpg etc)). Evidence can also be sent via email (to disabilitydyslexia@chi.ac.uk) or via post (see postal address below).

Acceptable evidence of a disability or medical condition would be a letter from your GP, the consultant involved in your case, or any other health professional who is involved in your treatment. The letter should give details of the nature of your condition and how it might be likely to affect you on your studies. This information will then allow us to consider the support you are requesting. This also helps us to consider whether there are any further support issues.

Any information you send us is treated as confidential and will not be released to any other part of the University without your explicit consent.

If you have difficulty accessing and/or completing the online form please contact the Disability and Dyslexia Service directly on disabilitydyslexia@chi.ac.uk who will send you the form in document format. The form and associated evidence can be sent back by email (to the same email address) or by post to:

After you submit the form and supporting evidence we will contact you with details of what support we feel we may be able to offer. We may also invite you in to meet with a Disability Advisor if appropriate.

Some courses will require all applicants to complete a declaration of health. For example, for Teacher Training Courses (QTS) The Training and Development Agency (TDA) regulations require a declaration of health to assess your general health and fitness to teach. Applicants to Social Work are required to make a similar health declaration. A disability is rarely regarded as a barrier to teaching.
